Foundations Health Assessment
The Foundations Health Assessment offered by Balanced Living Functional Medicine is designed to provide wellness education, foundational laboratory interpretation, and lifestyle-oriented functional medicine guidance for eligible clients.
This service is currently available only to individuals residing in the state of Ohio.
The Foundations Health Assessment may include:
review of foundational laboratory markers
wellness-focused recommendations
lifestyle guidance
supplement recommendations
educational interpretation of health patterns
This assessment is intended for educational and wellness purposes and is not intended to replace emergency medical care, urgent care, or ongoing primary care services.
Participation in this assessment does not establish an ongoing primary care relationship.
Laboratory testing, if needed, is purchased separately and is not included in the assessment fee.
Clients may submit recent laboratory testing for consideration; however, Balanced Living Functional Medicine reserves the right to determine whether submitted laboratory results are appropriate and sufficiently current for review.
The Foundations Health Assessment focuses on foundational health patterns and may not include detailed interpretation of every marker included in extensive laboratory panels.
No guarantees are made regarding specific health outcomes, improvements, or results.
Clients are encouraged to maintain an ongoing relationship with a licensed primary care provider for routine and urgent medical care.
If you are experiencing a medical emergency, call 911 or seek immediate medical attention.
Modification of Terms
We reserve the right to modify these Terms at any time, and the changes will be posted on this page. Your continued use of the Site after any changes are made indicates your agreement to the revised Terms.
Governing Law
These Terms will be governed by and construed in accordance with the laws of the jurisdiction in which Balanced Living is located(state of Ohio), without giving effect to any principles of conflicts of law.
